Burnfield JM, Buster TW, Goldman AJ, Corbridge LM, Harper-Hanigan K (2016). Partial body weight support treadmill training speed influences paretic and non-paretic leg muscle activation, stride characteristics, and ratings of perceived exertion during acute stroke rehabilitation. Hum Mov Sci 47:16-28. Introduction: Training intensity has been emphasized as a critical parameter for assisting neuroplastic brain changes, however, clinicians frequently report that training patients at faster speeds during PBWSTT is “hard work.” The authors of this study attempt to determine whether increasing walking speed is accomplished solely through therapists/facilitators working harder, or versus the patient exerting greater muscle effort. Understanding the relationship between training parameters and physiologic demands during PBWSTT provides essential data necessary for the design of treatment interventions during acute rehabilitation. This study compared muscular activation, temporal gait parameters, and perceived exertion in acute stroke survivors at 4 different walking speeds. The authors report that at faster walking speeds, participants demonstrated greater peak and average muscle activation in the paretic limb (vasti, gastrocnemius, and medial hamstrings) in stance (for stable shock absorption during weight acceptance; controlled forward progression in single limb support) and swing (deceleration), as well as greater non-paretic medial gastrocnemius activity (to control forward progression and maximize step length.)
