(2016) Variable intensive early walking poststroke (VIEWS): A randomized controlled trial. Neurorehabil Neural Repair 30:440-450. (epub 9/3/15) Introduction: Conventional clinical interventions focus on ameliorating impairments and provide practice of multiple functional tasks at low cardiovascular intensities, and focus on normalizing gait patterns. Previous studies of high-intensity training post-stroke utilize forward treadmill stepping as their primary intervention, and may not demonstrate gains in measured locomotor outcomes, with poor carryover to nonwalking functional tasks. Motor control research indicates that introducing variability into task-specific practice increases errors performed during practice, with corresponding improvement in retention. Variable stepping training may recruit more expansive neural circuitries underlying different stepping tasks while still focusing on the fundamental task of walking. The authors of this paper suggest that if patients can adjust their locomotor strategies to correct errors during task performance, allowing or augmenting erros has the potential to improve task performance. In a previous pilot study, the authors implemented training strategies to maximize stepping practice in multiple, variable contexts, performed at high intensities, while encouraging and enhancing errors resulting in improved locomotor and non-walking function. In this study, they compared high-intensity variable stepping training to conventional physical therapy in subacute stroke survivors. The high-intensity variable stepping training incorporated in the experimental intervention incorporated tasks of increasing difficulty, with applied perturbations to augment errors, similar to studies that target specific locomotor tasks in people with neurological impairments.
