Webinar Description

Excellent outcomes for adults who experienced early standardized treadmill training during acute phase of recovery from stroke: A case series.McCain KJ, Smith, PA, Polo FE, Coleman SC, Baker S. (2011).Top Stroke Rehab. 18(4):428-36. Summary by Nechama Karman, PT, MS, PCS A convenience sample of 18 people post-stroke (12 men, 6 women; 28-77 years (mean 53.9y); 2 cortical, 16 mixed/subcortical stroke, with an average of 3 co-morbidities; Motor recovery score (STREAM) range 0-75, (mean 43.61 +19.16)) admitted to Baylor Inst. For Rehabilitation participated in a standardized task-specific gait training program. The early, standardized treadmill training protocol (described in McCain, 2008) was initiated prior to over-ground gait training. Participants spent 30 minutes of their daily allotted therapy time gait training, with an additional 30 minutes dedicated to non-gait activities. BWSTT was performed with an ankle-foot orthosis (AFO), and began at .7pmh with 30% BWS. BWS was decreased in increments of 5% and speed was increased in increments of .1mph. Over-ground gait training commenced when a participant was able to walk on the treadmill for 3 minutes at .8mph with <10% BWS and < minimum assistance to advance the hemiparetic limb. Assistive device was limited to a single point cane and the AFO. Outcome measures: for gait symmetry, speed, endurance, assistive device reliance, and falls were collected between 6 and 10 months post-stroke for all participants, and included: Instrumented 3D gait analysis: 10 trials averaged. Markers bilaterally over ASIS, PSIS, medial knee epicondyle, lateral knee epicondyle, medial malleolus, lateral malleolus, head of 3rd metatarsal, posterior heel, lateral mid-thigh, and mid-lower leg. Participants walked on walkway with least amount of assistance required for safety, without devices (if this was safe). Single limb stance Double limb stance Swing time Speed Walking speed: six-minute walk test (6MWT)- with AFO, Assistive device if used in community; instrumented gait analysis Walking endurance: six-minute walk test (6MWT)- distance Falls history interview/questionnaire (caregiver, participant) Assistive device reliance: interview (participants/caregivers) and observation
