Webinar Description

The biomechanical parameters of gait underlying the long-term improvements seen in over-ground walking as a result of BWSTT have note been identified. Therefore, selected participants in the STEPS trial were stratified into either a high-response group (> .08 m/s increase in walking speed) or a low-response group (< .08 m/s increase in walking speed) on the basis of the magnitude of the change in self-selected walking speed between the baseline and post-intervention sessions. This study included kinematic, kinetic and muscle-activation variables to provide evidence regarding the gait parameters associated with improved walking speeds, and an indication of how those changes were achieved. Design: This was prospective, between-subjects design. (15) Subjects were 9 months to 5 years post-stroke with self-selected walking speeds < 1.0m/s (with assist of a single person, with or without assistive device or AFO). 7/15 subjects were classified in the high-response group, and 8/15 were classified in the low-response group. While there were no statistically significant differences between the groups with respect to age, sex, time since stroke, intervention group, baseline walking speed or assistive device use, there was a statistically significant difference in baseline LE Fugl-Meyer score between groups, with the high-response group having higher baseline scores. Baseline walking speed was higher (but not statistically significant) in the high-response group, and assistive device use higher at baseline in the low-response group Intervention: Included BWSTT (20 minutes, in 4 bouts of walking at 2 mph) twice a week for 6 weeks, along with one of 3 additional interventions (UE ergometry, resisted LE cycling or LE strengthening using PREs) an additional 2 days each week, for a total of 24 intervention sessions. All interventions were conducted by a licensed physical therapist.
