Rental Credit Application
A credit check must be completed for anyone planning on renting a LiteGait device.
If you plan on renting the LiteGait4Home, you will first need to complete a credit check. You can view and download the credit application form here. You can download, fill out everything and then email it to our accounting team. If you would prefer to provide your social security number over the phone, simply fill out everything but your SSN, email it to accounting, and then call our accounting department afterwards to provide it. Completing a credit check will usually take 1-3 hours.
If your credit check doesn't clear, we may still be able to rent it to you! It may just involve a higher upfront or monthly rental costs. We will work with the purchaser and determine the best course of action case by case.
